This policy explains how consumer health data may be collected, used, shared, and protected under Washington law.
Washington consumer health data. This separate policy describes practices for consumer health data that may be subject to Washington’s My Health My Data Act. It does not replace our Privacy Practices for protected health information.
Consumer health data we may collect
Washington law defines consumer health data broadly. In connection with our public website and service inquiries, information may qualify as consumer health data when it identifies or can reasonably be linked to a person seeking behavioral health or ABA services.
- Contact and identity information: name, email address, phone number, city or ZIP code.
- Service-seeking information: age-group selection, service interest, coverage category, preferred contact method, and the fact that a person is seeking information about ABA or behavioral health services.
- Technical information: IP address and basic website/server logs, to the extent those data are linked to a health-service inquiry or otherwise qualify as consumer health data.
Our public forms intentionally do not request diagnoses, medication information, behavior descriptions, medical history, or treatment records.
Why we collect and use it

- To contact you using your selected contact method.
- To determine whether we serve your geographic area and discuss availability.
- To discuss insurance or coverage-related next steps at a high level.
- To protect the security and integrity of our website and systems.
- To comply with applicable law.
Sources of data
We collect consumer health data directly from you or from a parent, guardian, or authorized representative who contacts us. We may also receive information from a professional referral source when authorized and appropriate. Basic technical information may be generated by our web server or hosting provider.
How data may be shared
We do not sell consumer health data. We do not share consumer health data for targeted advertising. We may allow processors and service providers to process information only as needed to operate the website, deliver form submissions and email, provide information technology or security support, or otherwise fulfill the service you requested.

- Website hosting and infrastructure providers.
- Email delivery and communication providers.
- Information technology and security service providers, when engaged.
- Professional advisers or authorities when disclosure is required or permitted by law.
envision Behavioral Health, LLC does not currently identify any separate commonly branded affiliate that receives consumer health data through this website. If that changes, this policy will be updated as required.
Your Washington rights
Subject to applicable law and verification, you may have the right to:
- Confirm whether we are collecting, sharing, or selling consumer health data about you and access that data.
- Receive information about third parties and affiliates with whom data has been shared or sold, where applicable.
- Withdraw consent to future collection or sharing when processing is based on consent.
- Request deletion of consumer health data, subject to applicable exceptions and permitted timing for archived or backup systems.
- Appeal a refusal to act on a request.

Describe the right you want to exercise and provide enough information for us to reasonably authenticate the request. An authorized agent or personal representative may make a request when permitted by law and may be asked to provide proof of authority.
