Notice of Privacy Practices
How we use, share, and protect health information.
This Notice explains your rights and how envision Behavioral Health may use or disclose protected health information.
Your rights
You have rights regarding protected health information. Depending on the circumstances and applicable law, these include the right to:
- Inspect or obtain a paper or electronic copy of your health record.
- Ask us to correct health information you believe is incorrect or incomplete.
- Ask us to communicate with you in a specific way or at a specific location.
- Ask us to limit certain uses or disclosures. We are not required to agree to every restriction, but we will follow restrictions we are required to honor.
- Request that information about care you paid for fully out of pocket not be disclosed to a health plan for payment or health care operations, when applicable and unless disclosure is required by law.
- Receive an accounting of certain disclosures of your health information.
- Receive a paper copy of this Notice at any time.
- Choose someone who has legal authority to act for you.
- File a privacy complaint without retaliation.
Your choices
For certain uses and disclosures, we may ask for your written authorization or give you choices, such as some communications with family or others involved in care, certain marketing activities, or other uses not otherwise permitted by law. You may revoke an authorization in writing, subject to actions already taken in reliance on it.
Uses that generally require written permission
We will obtain your written authorization before using or disclosing protected health information for purposes that require authorization under HIPAA, including the sale of protected health information, most uses of psychotherapy notes if we maintain any, and certain marketing communications.
How we may use and disclose health information
Treatment
We may use and share health information to provide, coordinate, or manage ABA and related health care services and to communicate with other health professionals involved in care when permitted.
Payment
We may use and share health information to bill for services, obtain authorization, determine benefits, and receive payment from health plans or other responsible payers.
Health care operations
We may use and share health information for activities needed to operate the practice, such as quality improvement, supervision, training, credentialing, compliance, auditing, business planning, and certain administrative functions.
Other permitted or required disclosures
HIPAA and other laws may allow or require disclosures for public health and safety activities, abuse or neglect reporting, health oversight, workers’ compensation, law enforcement or judicial proceedings, research under applicable safeguards, organ donation, coroners or medical examiners, and other legally permitted purposes.
Business associates
We may use service providers that perform functions involving protected health information on our behalf. When required, those business associates must agree by contract to safeguard the information and use it only as permitted.
Substance use disorder records
envision Behavioral Health does not provide a substance use disorder treatment program. To the extent we receive or maintain substance use disorder patient records that are subject to 42 CFR Part 2, we will not use or disclose information from those records in investigations or proceedings against the patient unless permitted by applicable law.
Our responsibilities
- We are required by law to maintain the privacy and security of protected health information.
- We will notify affected individuals as required if a breach occurs that may have compromised the privacy or security of their information.
- We must follow the privacy practices described in the Notice currently in effect.
- We will not use or disclose health information for purposes not described in this Notice unless you authorize us in writing or another law permits or requires it.
Washington and other privacy protections
Washington law may provide additional protections for certain health care information or may impose additional limits on disclosure. When a state or federal law provides greater privacy protection, we follow the law that applies to the information and circumstances.
Changes to this Notice
We may change the terms of this Notice. A revised Notice may apply to health information we already have as well as information we receive in the future. The current Notice will be available on our website and upon request.

If you have questions, want to exercise a privacy right, or believe your privacy rights have been violated, contact envision Behavioral Health using the phone or email below. We will not retaliate against you for filing a complaint.
